  • Submit the application.
  • Mail the payment to CDS at the address listed below.
  • Download, print and fill the CUSIP/ISIN request form (PDF)
  • Fax or mail the completed form to CDS.
  • Mail the payment and two copies of supporting documentation to CDS at the address listed below.

The following documentation, in draft form where applicable, must be included with the request. Please include two copies of documents submitted by mail.

Note: For medium-term notes (MTN) programs, a shelf prospectus is required.


Final copies of the above documentation, where applicable, must be filed with CDS. Incomplete applications or applications not supported by appropriate documentation will not be processed.
